Therapy Consent & Terms

This document sets out how we’ll work together. Please read carefully and confirm your consent by signing at the end (typing your name/date is fine for online sessions).

1) Service & Approach:

I provide psychotherapy/coaching-informed therapeutic support focused on calm, clarity and relational wellbeing. Sessions are collaborative, trauma-aware and goal-oriented. We can discuss whether therapy or coaching is the best fit for your needs.

2) Sessions:

·       Length: 50 minutes (standard).

·       Location: secure video (Google Meet) or by arrangement in person.

·       Frequency: usually weekly/fortnightly; we will review regularly.

3) Fees & Cancellations:

·       AXA-funded sessions: AXA pays £85 per session to recognised providers. If your policy has an excess, you agree to pay that excess directly to me when due.

·       Private/self-pay sessions (outside insurance): £150 per 50 minutes (confirm before booking).

·       Cancellations/rescheduling: please give at least 24 hours’ notice. Late cancellations or missed sessions may be charged in full; insurers typically do not fund no-shows.

4) Confidentiality (and its limits):

Your information and what you share in sessions are confidential. I will not disclose content without your consent except where there is a legal/professional duty to do so, including if I believe you or someone else is at risk of serious harm; safeguarding of children or vulnerable adults; prevention of terrorism or serious crime; money laundering; court order; or as required by my professional body/insurer. I receive clinical supervision where your identity is protected.

5) Records & Note‑keeping:

·       I keep brief clinical notes and basic contact/administrative information. Notes are factual and proportionate.

·       Records are stored securely and retained for up to 7 years after our work ends (or until age 25 for work begun with a minor), then confidentially destroyed.

6) Telehealth Conditions:

·       Please ensure you’re in a private, quiet space with a stable connection. Use headphones if possible.

·       If the connection drops, I will attempt to reconnect; if unable, I will call/email to reschedule the remaining time.

7) Communication Boundaries:

·       Email/text is for scheduling and brief updates only (not for therapy-by-email).

·       I aim to reply within 1–2 working days. I do not offer an emergency service.

·       For crisis or urgent risk, contact NHS 111, Samaritans 116 123, your GP, or emergency services (999).

8) AXA Funding — Consent & Information Sharing:

·       If your sessions are funded by AXA, you consent to me holding and using your membership number and pre‑authorisation code to invoice AXA for sessions delivered within the authorised dates.

·       I will share only the minimum necessary administrative information with AXA (dates attended, session type/length, fees, codes). I do not share detailed session content.

·       For renewals, AXA may require a brief progress summary (non‑clinical) to justify further sessions; we will agree it together. You can review what is shared.

9) Endings & Reviews:

·       You may end therapy at any time. I encourage a planned ending session where possible.

·       I may suggest pausing or signposting if therapy is not the right level of support for your needs.

10) Consent:

By signing below, you confirm that you have read and understood these terms and consent to proceed with therapy under these conditions.
